Who we are

ConradCare is a specialist supported living service providing high-quality support for persons between the ages of 16-65 across Greater London, Berkshire and Surrey. We pride ourselves in delivering high-quality person-centred care.

Supported Living for Adults

​

Our adults service is dedicated to creating a peaceful and therapeutic environment for adults who are living with learning difficulties. We closely collaborate with individuals currently residing in secured units, who have been determined to be ready for the next phase of their recovery journey - transitioning into the community.

​

Our service is driven by outcomes, with a strong focus on empowering our service users to eventually secure their own tenancies with minimal support, whenever possible. We believe in promoting independence and providing the necessary support for individuals to thrive in their own homes.

Service breakdown:

  • Supported living for adults with Mental Health, Forensic Mental Health, Learning Difficulties & Autism

  • 24-hour staff presence

  • Nurturing environments accommodating a maximum of three service users to ensure highest level of support

  • Recovery and outcome-based support

  • One-to-One or Two-to-One support as appropriate

  • Therapeutic intervention by selected providers

  • Person Centered, empowering approach

  • Positive Behavior support

  • Pathways to own tenancies where appropriate

  • Life skills training

  • Social and community integration

  • A service user mix that is sensitive to gender, age and risk profile

Supported Living for 16+ 

 

We offer our supported living service to young people aged 16+ who are in the care system or transitioning out of it. We have a strong history of delivering outcome-based support to individuals with Learning difficulties and Autism, working closely with various local authorities in London, Surrey, and Berkshire.

Our service is specifically designed to help these young people develop essential independent living skills and align with their leaving care pathway plan. We are truly proud of the progress our young people make while under our care.

  • High quality accommodation in a safe and secure environment.

Our Experience

  • Extensive quality experience in healthcare  within NHS (Intensive Psychiatric Unit) as a recovery and engagement worker.

  • Supported Living Services Manager

  • Adult and children residential Care Manager

  • All the above have provided great sense of reward and fulfilment, supported in identifying critical deficiency in the execution and approach towards person-centred healthcare and outcome based led services which led to founding CONRADCARE, there to be the catalyst in DOING CARE RIGHT.

ConradCare Ltd is currently completing the regulatory process to extend our work into Residential Homes services to deliver our services and outcomes to older client base.
